How to apply for an international vaccination passport
There are several ways to prepare a document.
1. If all vaccinations are done in our clinic
If a child receives all vaccinations from us, we start keeping an international certificate automatically from the first vaccination .
2. If vaccinations were done in other medical facilities
If all or part of the vaccinations were not done by us, but the certificate was not issued or was lost, we will need medical certificates with doctor's stamps about previously performed vaccinations.
3. Registration at the clinic
In Kyiv or Kharkiv, you can make an appointment, bring medical certificates, and quickly obtain an international vaccination certificate directly at the clinic.
4. Remote registration
You can also issue the document remotely. Send us scans or clear photos of certificates of each vaccination. We will enter the data into the certificate, certify the document and send you a scanned copy by e-mail. In Ukraine, we can also send a paper international vaccination certificate by mail.
What vaccinations can be included in the certificate?
All vaccinations for which there is medical confirmation can be included in the international vaccination passport:
- vaccination against COVID-19
- flu vaccination
- mandatory childhood vaccinations
- other recommended vaccinations
